A 62 year-old patient of mine lost his job of 30 years, could not afford medicines for his lung disease, and ended up in the intensive care unit for a long hospitalization a few months later. Our fragmented system leaves too many cracks through which my patient can fall.

Had a patient that was unable to afford insurance that was diagnosed with stage IV brain cancer. He had to make the choice of bankrupting his and his family's finances or getting adequate life-prolonging treatment. Affordability and accessibility have to be included in health care reform.

These End Stage Renal Dialysis patients are uninsured and unable to schedule Chronic Renal Dialysis; thus, waiting 1-2 weeks until there disease progresses to requiring acute renal dialysis as an inpatient. A less than optimal management choice for their disease.

A few years ago, I saw a school-age child for her physical exam. I heard a murmur that required evaluation by a cardiologist. Mom was concerned about being able to afford the co-pay for a specialist--despite working three jobs to make ends meet. This type of scenario is a travesty.
